WebI've heard its a glitch and sometimes companions just disappear when dismissed, never to be seen again unless they appear in a quest later. Oh well, at least I saved right before I dismissed her. I haven't had this problem yet (seems pretty common though), but Beth should have given a easier way to find companions.
